Rest days: I've been religious about rest days for months but I've just been out for a short slow mile to start the day and it reminded me how good a daily run streak can feel with a short slow trot on the days you don't have anything else planned.How important are full rest days?2.) Xmas events are starting to interfere with my training schedule. If I am having to cut sessions my sense is I should prioritise a weights session over a run but just looking for some advice on balancing.Melissa3.) I have been told over the years several times by PTs doctors and read it online that tight hips and hip flexors will create a pelvic tilt that pulls on the hamstring and will aggravate PHT or may have even caused PHT. Is this accurate information?4.) How do you know when to increase your weights in your strength training program? It has been several weeks/months since I have increased any of my weights, but they still feel challenging. After my last flare-up, my baseline never returned to a 0 pain, and hovers between a PL1 or 2 and sometimes sitting for really long periods can be a bit uncomfortable again.

Q&A: Global PHT awareness/tendon strength assessment/rehab after recovery/mid-belly soreness

In today's episode, Brodie answers the following questions: Julia: Hi Brodie I have a general query about the fact that this condition is so difficult to treat within the physio/orthopedic community with seemingly only a handful of specialists (like yourself) who understand how to rehab it successfully. Is this a new 21st-century condition that has been amplified by our sedentary lifestyles and long hours of sitting at work? It just seems strange that no one had this problem back in the 80s and 90s when jogging/running became so popular.  Harvey: Hi Brodie, Maybe you could expand on how a PHT sufferer assesses whether they have a relatively strong or weak tendon. For example, a slight increase in a strength exercise or perhaps too much sitting in a given time period will cause a flareup and will often take 4-5 days for the tendon to settle back down. Trina: Rehabilitation exercises are great; usually doing the job to strengthen and “repair” if following doctor’s orders. However, once the hamstring and/or tendon no longer cause pain, is it expected that someone would continue the exercises, and if so, would it be less frequently, fewer reps, lower intensity, or something else? In other words, what is the post recovery protocol? Jen Johnson: Hi Brodie, my question is, how common is it to have pain in the "muscle belly" of the hamstring? Is this truly connected to this PHT injury? I occasionally have pain at the high tendon attachment but it is rare. I have really stalled in my rehab and I am wondering if it could be something else. I should clarify, that in the beginning, it was more prominent in the attachment but not now. Arthi's PHT success story

Q&A: Running as rehab/muscle building/PHT race preparations

Progressive loading exercises didn't work and eventually, I gave up. Nothing seemed to work. I've now started running again building up very slowly (started with 3x1 minute running and a minute walk between). I can now run 3.5 miles pain-free. In your opinion is this something that could continue to work for me and has anyone ever recovered from PHT in this way?" “Is there any technique to build LL muscle with this tendinopathy? PHT about 3 years. Finally, a year ago I found a PT which made me workout through pain (at no more than 4-5) with controlled movements, which has helped me be able to squat, deadlift, and some other movements at 75%-85% of my 1RM, which has improved my body quite a lot. I still find limits in what I can do or how much stress I can put into my workout. So please some advice on this subject would be greatly appreciated!” “Would love to hear more about rehabbing PHT while training for a race. I'm scheduled to run Boston in April and have been rehabbing since December 3rd. I haven't yet introduced speed back in but slowly building up mileage. Here is their submission when asked what their biggest insights and revelations were during Proximal hamstring tendinopathy rehab: "My main learning points from my 2.5 years would be: Don’t think that PHT will go away by completely resting. Pain will decrease (with rest) but once you attempt to return to activity, then you’ll have more than PHT to worry about. Seek input from someone who has experience in treating this condition. Keep notes on what activity you do and reactions to this, both rehab and what other daily activities might impact. Do not accept corticosteroid injection if suggested. Here is their response when Brodie asked what really helped their recovery: "Well definitely the exercises you said to do! And made sure I stuck to doing them. I stopped doing the glute bridges etc and focussed on increasing weight-bearing ones. I had read about it and self-diagnosed at first and thought I knew what exercises to do but didn’t know how many to do or how to progress through the exercises. I didn’t realise that I could have kept running. And if I could start again, exactly how. I avoided long sitting and if I sat, I used a cushion. I found other ways to exercise like swimming which I found I could do without making it a flare-up. Definitely need to find a physio that knows how to diagnose PHT and how to treat it. I definitely needed someone to relate the treatment to my particular condition and what I was able to do or not do. It’s not a quick fix so don’t give up! I still have some runs where I feel it but the next day it’s fine again". Success Story: Todd Charnetski 11 years of PHT

The 3 stages of Proximal Hamstring Tendinopathy Rehabilitation

On today's episode, Brodie dives into the stages of PHT rehab. We first touch on the responsibility of pain during rehab exercises, and how we can interpret productive pain from unhelpful pain. Next, we discuss the 3 stages of rehab. Stage #1 includes how to settle down pain from a reactive flare-up, including the appropriate use of isometrics, daily load modifications and medications. Stage #2 covers different levels of strengthening exercises. Brodie explains how the tendon can start 'out of compression' and slowly build on compressive loads throughout the rehab process. Certain exercises include hamstring curls, Swissball bridging, swissball rollouts, deadlifts, lunges & squats. Understanding Proximal Hamstring Tendinopathy
