The Importance of Coronary Artery Calcium Scoring

The past week has seen the untimely passing of two prominent Australian figures, Shane Warne and Kimberly Kitching. Both had a fatal heart attack, and both were 52 years old.In this episode, Peter and Stuart talk about the importance of early identification of potential risk of heart attack. A key test used in this area is the Coronary Artery Calcium (CAC) Score test.The CAC score is the international gold standard for predicting heart attacks in patients over the age of 40. This quick, painless procedure measures the amount of calcium in the walls of the arteries that supply the heart muscle, using a special scan of the heart.The CAC score looks at the overall cardiac plaque present, which can help predict the risk of a cardiovascular event, like a heart attack or stroke, in the future. Pericarditis

Pericarditis is the swelling of the membrane around the heart.It has had quite a lot of coverage in the media lately as it has been identified as a possible, although very rare, side-effect of the mRNA Covid vaccines. You are, however, more likely to develop pericarditis as a result of Covid (11 people per 100,000) than as a result of the mRNA vaccine (2.7 people per 100,000).In this episode, Peter and Stuart discuss a recent case study who presented at Heart HQ with acute pericarditis after a respiratory tract infection. Mental Health, Psychosocial Factors and Cardiac Risk

Left Atrial Appendage Closure

In this week's episode, Peter and Stuart discuss Left Atrial Appendage (LAA) Closure.The LAA is a small thumb-sized pouch near the top of your heart. If you suffer from a condition called atrial fibrillation, blood can stagnate and form clots inside your LAA, which is a major cause of stroke.If you have a history of atrial fibrillation, an LAA closure procedure may be an alternative to taking blood thinning medication long term. Dual Antiplatelet Therapy, Anticoagulants and Surgery

In this week's episode Peter and Stuart discuss dual antiplatelet therapy (DAPT), anticoagulants and surgery.They are often asked by doctors whether patients with previous stenting or acute coronary syndrome can temporarily stop DAPT before and after surgery. Stuart discusses how this may impact patients, specifically as it relates to non-cardiac surgery, both urgent and non-urgent.Peter also takes a closer look at anticoagulants. Temporarily ceasing these medications can increase thromboembolic risk, whereas continuing these medications can increase the risk of perioperative bleeding. Aspirin as Primary Prevention and Bicuspid Aortic Valves

It used to be common that patients were given aspirin as a preventative measure. However, while it may help prevent heart attack and strokes in some people, it can lead to very serious side-effects such as internal bleeding.In this week's podcast, Peter and Stuart take a look at some of the studies that have been carried out into aspirin for primary prevention. They also discuss how clinicians, nurses and patients being better able to manage diabetes, lipids and blood pressure has led to much better outcomes for patients than daily aspirin use.They also chat about a patient of Stuart's who presented with a very large ascending aorta aneurism. He had a bicuspid aortic valve (an aortic valve that has two flaps (cusps) instead of three) and while he was relatively asymptomatic, it was discovered that he had severe aortic valve regurgitation. Statin Nocebo and Hypertrophic Cardiomyopathy (HCM)

In this week's episode, Dr Peter Larsen and Dr Stuart Butterly discuss Statin Nocebo and Hypertrophic Cardiomyopathy (HCM).They explain that statins are an important primary and secondary prevention measure. A big issue for patients is the side-effects from statins. However, Stuart discusses a study that revealed that it might not be the statin, but the act of taking medication that can result in these symptoms (known as a 'nocebo effect').
